Hospice care inside their community, no move required. Community teams and families reach us on one line, any hour.

WHEN TO CALL

When It's Time to Add Hospice

The community keeps doing what it does well. Hospice adds what the circumstances are asking for, right in their own room.

SIGNS:

  • A resident is declining despite attentive care.
  • Hospital trips are starting to add up.
  • The family is asking what comes next.
  • The community suggested hospice, and you're choosing one.

LOGISTICS

How It Works Inside a Community

Assisted living communities and personal care homes don't need a contract to work with AIP. Directors and community nurses call us directly, families do too, and every call gets a response within thirty minutes, any hour.

The partnership is simple: the community keeps providing the daily life it always has, and AIP adds the hospice layer, the nursing, the symptom management, the medications and equipment, folded into the rhythm that already exists.

Residents keep their room, their neighbors, and their routine, and the faces AIP sends stay consistent, visit after visit. Your staff knows exactly who's walking in, and so does the resident.

For the family, it stays plain and close: updates in words that don't need translating, a chaplain and social worker for the whole family, and support that continues through the year after. If the community handed you a few hospice names to choose from, one call answers everything.

Coverage is clean: Medicare's hospice benefit covers AIP's care in full for those who qualify, completely separate from the community's own costs. We'll walk the family through exactly how the two fit together.

COMMON QUESTIONS FROM REFERRING TEAMS

Frequently Asked Questions

Straight answers for the people who send patients. For anything else, the referral line answers any hour.

How fast does AIP respond to referrals?

AIP responds to every referral within 30 minutes, any hour of any night. Admission completes the same day when clinicals and consents are ready, and the first visit happens within one hour of signed consent.

How do I send a referral to AIP?

Referrals reach AIP through CarePort, by fax, or on the referral line. Send recent clinicals, a medication list, and the family's point of contact; AIP handles benefits verification, consents, and physician orders from there.

Can AIP provide GIP (inpatient) care?

Yes. AIP holds a standing GIP contract with the Methodist Hospital System, with credentialed physicians writing orders in-system, so inpatient-level hospice can begin without a contract scramble.

Is AIP VA-contracted?

Yes. AIP is one of a small number of VA-contracted hospices in San Antonio, and also holds contracts with VA-contracted nursing homes in the area.

How quickly can hospice care begin?

AIP says so, plainly. If a patient isn't yet hospice-appropriate, we'll tell you and the family what we're seeing, and when home health is the better fit for now, we'll say that too.
