Hospice Handoffs for Home Health Agencies

When your patient declines toward hospice, the handoff decides how the family remembers your agency. We make it fast, seamless, and kind.

FOR YOUR PATIENTS

A Transition That Doesn't Feel Like One

Your nurses see the moment coming before anyone: declining despite good care, goals shifting from recovery to comfort, a family starting to ask different questions. When that moment arrives, one call moves everything. We respond within thirty minutes, any hour of any night, and admission often completes the same day, with the first visit within an hour of signed consent.

For the family, nothing lurches. Care continues in the same home, the transition is explained in plain words, and the team that takes over is built to be unhurried: nurses at about half the typical hospice caseload, the same faces visit after visit. The last thing your agency did for them was hand them to people who treat them well, and families remember that.

LOGISTICS

How to Send a Referral

Call the referral line or fax the packet: recent clinicals, medication list, and the family's point of contact. We take it from there, benefits verification, physician orders, consents, and we keep your team informed through admission. If a question needs a clinical answer first, our administrator reviews personally, and you'll hear back the same day.

COMMON QUESTIONS FROM REFERRING TEAMS

Frequently Asked Questions

Straight answers for the people who send patients. For anything else, the referral line answers any hour.

How fast does AIP respond to referrals?

AIP responds to every referral within 30 minutes, any hour of any night. Admission completes the same day when clinicals and consents are ready, and the first visit happens within one hour of signed consent.

How do I send a referral to AIP?

Referrals reach AIP through CarePort, by fax, or on the referral line. Send recent clinicals, a medication list, and the family's point of contact; AIP handles benefits verification, consents, and physician orders from there.

Can AIP provide GIP (inpatient) care?

Yes. AIP holds a standing GIP contract with the Methodist Hospital System, with credentialed physicians writing orders in-system, so inpatient-level hospice can begin without a contract scramble.

Is AIP VA-contracted?

Yes. AIP is one of a small number of VA-contracted hospices in San Antonio, and also holds contracts with VA-contracted nursing homes in the area.

What if my patient isn't hospice-appropriate yet?

AIP says so, plainly. If a patient isn't yet hospice-appropriate, we'll tell you and the family what we're seeing, and when home health is the better fit for now, we'll say that too.
