Hospice added to nursing home care, coordinated daily with your staff. Families and facility teams reach us on one line, any hour.
WHEN TO CALL
Your staff usually sees it first. When they do, here's what adding AIP looks like.
SIGNS:

The same bed, the same window, the same hallway friends.
Daily Coordination With Your Nurses
Two teams, one plan, updated every visit.
Contracted Across San Antonio
AIP holds nursing home contracts area-wide, including with VA-contracted homes.
Comfort managed early helps keep them out of the hospital.
The Family, Fully Supported
Chaplain, social worker, plain-word updates, and support that continues after.
Nurses and aides in the language your residents speak.

LOGISTICS
Nursing homes require hospice contracts, and AIP holds them across San Antonio, including with VA-contracted homes. If we're not yet contracted with yours, that conversation starts with one call.
Referrals come by fax or the referral line, and every one gets a response within thirty minutes, any hour of any night. When clinicals and consents are ready, admission completes the same day.
Inside the building, it's two teams on one plan. Our nurses and aides chart their visits, keep your nurses in the loop daily, and handle the hospice side, the symptom management, the medications, the equipment, without adding work to your floor.
The quiet result is the one your building gets measured on: comfort managed early means fewer transfers, and residents spending more of their days in their own home instead of a hospital hallway.
Families stay close the whole way: updates in plain words, a chaplain and social worker for the whole family, and the same AIP faces at the door, visit after visit. Families choosing between hospices can call the same line and ask anything.

COMMON QUESTIONS FROM REFERRING TEAMS
Straight answers for the people who send patients. For anything else, the referral line answers any hour.
AIP responds to every referral within 30 minutes, any hour of any night. Admission completes the same day when clinicals and consents are ready, and the first visit happens within one hour of signed consent.
Referrals reach AIP through CarePort, by fax, or on the referral line. Send recent clinicals, a medication list, and the family's point of contact; AIP handles benefits verification, consents, and physician orders from there.
Yes. AIP holds a standing GIP contract with the Methodist Hospital System, with credentialed physicians writing orders in-system, so inpatient-level hospice can begin without a contract scramble.
Yes. AIP is one of a small number of VA-contracted hospices in San Antonio, and also holds contracts with VA-contracted nursing homes in the area.
AIP says so, plainly. If a patient isn't yet hospice-appropriate, we'll tell you and the family what we're seeing, and when home health is the better fit for now, we'll say that too.
